Florida Residency: Care through The Whole NP is currently available only to Florida residents. Please confirm your residency prior to scheduling.

Patient Questionnaires: Complete your new patient intake forms in the portal so we can better understand your health history, current concerns, and long-term goals.

Consent Forms: Please review and sign all required consent documents through the portal prior to your visit.

Medical Records: Upload any recent or relevant labs, imaging, or medical records directly to your portal account before your appointment.

Commitment to Lifestyle Interventions: Our care model is rooted in proactive, lifestyle-centered healing. We ask that all patients come prepared to engage in nutrition, movement, mindset, and restorative lifestyle practices as part of their care plan.

***New Patient Inquiries: We are so abundantly grateful for the trust and support of this community. Because TheWholeNP is intentionally designed as a small, relationship-centered practice, we are only able to care for a limited number of patients at any given time. This allows us to provide the depth, presence, and personalized care that each individual deserves.

At this time, we do maintain a waiting list for new patients. As current patients transition into maintenance phases and pause their active care, openings become available. We are typically booking approximately six to eight weeks out for new patient establishment visits, and all inquiries are thoughtfully added to our waiting list.
